ON-SITE Pre-Enrollment Process for Tertiary Education Program
For BS in Nursing and BS in Accountancy incoming first year students who desire to enroll must undergo an Entrance Examination.
Note: For old/continuing students or incoming first year (except for Nursing and Accountancy programs), you may proceed to Step 3.
Step 1: PAY AT THE CASHIER FOR TAKING THE ENTRANCE EXAM
Pay at the cashier for the Entrance Examination and proceed to the Guidance Office (located beside the Grade School Bldg.). Show the receipt to the Guidance staff to start your entrance examination test.
Step 2: TAKE THE ENTRANCE EXAM AT THE GUIDANCE OFFICE
After paying, show the receipt to the Guidance Staff. After taking the exam, fill out the form with your contact details for the enrollment process.
Step 3: GETTING AND FILLING OUT THE FORM
Go to the Program Heads’ Office on Room 111, Annunciation Bldg. and ask the form from your Program Head and fill it out to start your pre-enrollment.
Step 4: PAYMENT AT THE CASHIER
After the Program Head checks the form, you proceed to the cashier for the reservation fee (minimum of ₱1,000).
Step 5: LISTING FOR PRE-ENROLLMENT
After paying the reservation form, go back to the Program Head for the verification and listing of your name in the list of students on the program.
ON-SITE Application Process for Tertiary Education Program
Step 1: GET AND FILL OUT YOUR ENROLLMENT FORM
After taking the exam and getting the result (for incoming first year and transferee BS in Nursing and Accountancy students only) Get and fill out your Enrollment form at Assumption Building, Room 101. The Admissions-in-charge will check your pertinent documents and the enrollment form.
Step 2: LOADING OF SUBJECTS
After the Admissions-in-charge checks the documents and signs your enrollment form, you may proceed to the Program Heads’ Office, Room 111, Annunciation Bldg. to load your subjects for your respective programs.
Step 3: ENCODING OF SUBJECTS
After the Administrative Staff / Teacher-in-charge / Program Heads checks the completeness of your subjects/courses and signs your enrollment form, you may proceed to the Registrar at the Administrative Building to encode your subjects/courses.
Step 4: PAYMENT AT THE CASHIER
After the Registrar / Registrar Staff encodes your subjects/courses and signs the enrollment form, you may proceed to the cashier just across from the Registrar’s office for your payment (down or full payment). The cashier will issue your official receipt, assessment, and signs your Enrollment form.
Step 5:
For new students: PROCEED TO ID REPRODUCTION
For old students: GET THE ID VALIDATION STICKER FROM YOUR RESPECTIVE PROGRAM HEADS
