- Police Clearance/Barangay Clearance (for male students)
- Entrance Test Result for Placement
- 1"x1" Picture for Examination Permit
- 2"x2" Picture for the Guidance Office File
Additional Admission Requirements for Nursing Program
- Stool Exam
- Urinalysis
- CBC
- Chest X-Ray
- Hepa Screening
- Medical Certificate
Academic Standards
- Students Academic Load
- Regular student - 18 and above number of units
- The number of loads allowed per semester as indicated in the course curriculum.
- Students should take the required subjects by curriculum year. Advance subjects may be taken only after passing the basic and corresponding pre-requisite subjects.
